Er Miao San, also known as Two-Marvel Powder, is a classical Chinese herbal formula made from just two herbs: Phellodendron bark (Huang Bai) and Atractylodes lancea rhizome (Cang Zhu). It is usually taken as a powder, in roughly equal parts. Although it is a formula, it is sold and dosed as a single drug preparation, so treat it as one product with one combined set of risks.
In traditional Chinese medicine it is used for what practitioners call "damp-heat in the lower body": hot, red, swollen or aching knees, ankles and feet, heavy weak legs, itchy weeping skin on the lower limbs, and thick vaginal discharge. Modern sellers often market it for gout and arthritis.
The honest evidence position: this is a long-standing staple of one tradition, but we could not identify published human trials of the formula itself. Traditional use tells you what people have done for centuries; it is not proof that the formula works.
Nobody has shown how Er Miao San works in people. What follows is theory plus laboratory findings, not proven action.
In traditional terms, Phellodendron bark is said to clear "heat and damp" from the lower body, and Atractylodes is said to dry "damp" and strengthen digestion. These are traditional concepts, not descriptions of body chemistry.
In the laboratory, berberine (the main bitter compound in Phellodendron bark) reduces inflammatory signals in cells and in animals, and lowers uric acid in some rodent gout models. Cell and animal results often do not repeat in people, so treat this as a starting point for research, not a reason to expect benefit.
You will most often meet Er Miao San as one of these:
Loose powder, the classical form, made by grinding the two dried herbs together.
Pills or capsules of the same powder, often labelled Er Miao Wan.
A decoction, meaning the herbs simmered in water and the liquid drunk, prepared by a practitioner.
Granules or "instant" extracts, where a concentrated water extract is dried onto a carrier and dissolved in hot water.
Extracts and raw powders are not interchangeable gram for gram. A concentrated granule is much stronger than the same weight of plain powder, so follow the label of the product in your hand.
Er Miao San is credited to Zhu Danxi, a Chinese physician of the Yuan dynasty, and appears in texts associated with his work in the 14th century. The name "Two Marvels" simply refers to its two herbs.
It became a base formula that later physicians expanded. Adding Achyranthes root gives San Miao San (Three Marvels), and adding coix seed as well gives Si Miao San (Four Marvels). These variants are still widely prescribed today.
Its standing is solid within Chinese medicine and it appears in standard formula textbooks. It has no parallel history in Ayurveda, Western herbalism or any other tradition.
See a doctor if a joint is hot, red and very painful and comes with a fever, because that can mean a joint infection, which is an emergency.
See a doctor if you get yellowing of the skin or eyes, dark urine, pain in the upper right side of your belly, or unusual tiredness. These can be signs of liver trouble, and you should stop taking the powder.
See a doctor if burning urination, back pain or fever develops, if vaginal discharge becomes smelly, bloody or painful, or if you take diabetes medicine and feel shaky, sweaty or confused.
Also get medical advice before starting if you take any regular prescription at all, and stop straight away if you develop a rash, swelling or breathing trouble.
Evidence grades and traditional-use labels are explained in our methodology. Nothing here is medical advice.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
