What is Di Huang (Rehmannia)? Di Huang (Rehmannia glutinosa) is a perennial flowering plant native to China whose tuberous root has been a cornerstone of Traditional Chinese Medicine for over 2,000 years . It exists in two pharmacologically distinct processed forms: Sheng Di Huang (raw or dried root), considered cooling and used for heat-related conditions, and Shu Di Huang (wine-steamed prepared root), considered warming and tonic in nature. These processing methods meaningfully alter the phytochemical profile and clinical applications of the herb. How does Di Huang (Rehmannia) work? The primary bioactive constituents—particularly catalpol and the rehmannioside glycosides—appear to exert hypoglycemic effects by modulating insulin signaling pathways and improving peripheral glucose uptake. Acteoside and polysaccharides contribute anti-inflammatory and immunomodulatory activity, while oligosaccharides may support the gut microbiome. Catalpol has also been studied for neuroprotective and osteogenic (bone-forming) properties in laboratory models, though human translation remains preliminary. What forms does Di Huang (Rehmannia) come in? In clinical practice and most human trials, Di Huang is rarely used as an isolated single herb—it is most commonly studied as part of the classical formula Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia), which complicates attribution of effects to Rehmannia alone. Standardized extracts providing defined catalpol percentages (commonly 0.5–1% ) are increasingly available in Western markets. Traditional decoction remains standard in East Asian clinical settings. Where it comes from: Di Huang has been documented in Chinese materia medica since the Shennong Bencao Jing (Divine Farmer's Classic of Materia Medica, circa 200 CE), where it was classified among the superior (tonic) herbs. It became the foundational ingredient of Liu Wei Di Huang Wan, formulated by Song dynasty physician Qian Yi (1032–1113 CE) and still among the most widely prescribed herbal formulas in East Asia today.

Helps

What does Di Huang (Rehmannia) help with? Type 2 Diabetes (blood sugar regulation): Multiple randomized controlled trials have examined Liu Wei Di Huang Wan (LWDHW)—a Rehmannia-dominant formula—as adjunctive therapy in type 2 diabetes. Several published trials in Chinese-language and international journals have reported reductions in fasting blood glucose and HbA1c when LWDHW was added to conventional antidiabetic medication compared to medication alone. The proposed mechanisms include improved insulin receptor sensitivity and modulation of hepatic gluconeogenesis, likely attributable in part to rehmannioside and catalpol fractions. Because these trials study a multi-herb formula rather than isolated Rehmannia, it is not possible to assign the full effect to Rehmannia alone. A systematic review published in Evidence-Based Complementary and Alternative Medicine (Xiong et al., 2013) evaluated multiple RCTs of LWDHW in type 2 diabetes and found generally favorable but methodologically heterogeneous results. Osteoporosis / Bone Density Loss: Rehmannia-containing formulas, particularly Zuo Gui Wan and Liu Wei Di Huang Wan, have been evaluated in clinical trials for postmenopausal osteoporosis. A randomized trial by Zhang et al. (published in the Journal of Ethnopharmacology, 2012) examined a Rehmannia-based formula in postmenopausal women and reported improvements in bone mineral density markers compared to control, with proposed mechanisms involving upregulation of osteoblast activity and modulation of estrogen-related signaling pathways. Laboratory evidence strongly supports catalpol as an osteogenic compound, but human trial evidence for isolated Rehmannia on bone density remains limited—most human data comes from multi-herb formulas. Anemia: Shu Di Huang (prepared Rehmannia) is a classical TCM treatment for blood deficiency, including iron-deficiency and anemia associated with chronic disease. It is typically combined with herbs such as Dang Gui (Angelica sinensis). The polysaccharide fraction of prepared Rehmannia has demonstrated hematopoietic-stimulating effects in preclinical models, potentially by supporting erythropoiesis in bone marrow. Human clinical evidence for Rehmannia's role in anemia is largely embedded in formula-based trials and traditional practice rather than isolated single-herb RCTs; evidence should be considered preliminary for this specific indication. Adrenal Insufficiency / Chronic Fatigue (TCM Kidney Yin deficiency): In TCM, Di Huang is the primary herb for 'Kidney Yin deficiency,' a pattern characterized by symptoms overlapping with adrenal insufficiency, fatigue, night sweats, and low-grade inflammatory states. Some clinical research has examined Liu Wei Di Huang Wan in patients with subclinical hypothyroidism and fatigue-dominant presentations, with modest positive findings. The mechanistic basis may involve modulation of the hypothalamic-pituitary-adrenal (HPA) axis and reduction of excess cortisol-driven inflammation, though direct HPA-axis evidence in humans from isolated Rehmannia is lacking. Autoimmune and Inflammatory Conditions: Acteoside and polysaccharide fractions from Rehmannia exhibit immunomodulatory properties, including suppression of pro-inflammatory cytokines (TNF-α, IL-6) in cellular and animal models. Clinical application in autoimmune conditions (such as rheumatoid arthritis and lupus) has been investigated in small Chinese clinical studies, typically using formulas containing Rehmannia. A review of TCM approaches to lupus nephritis has noted Liu Wei Di Huang Wan as one of the most commonly used formulas, though high-quality isolated RCT evidence for Rehmannia specifically in autoimmune disease remains limited and should be interpreted cautiously.

This app is so cool! I've always wanted there to be a hub for all NATURAL remedies and preventative options but l've always had to do loads of digging to find the info I wanted for different natural products. HealisticMD has all the info I need in one place. It gave me various natural supplemental options for a condition I looked up, including what each remedy does and how much is usually recommended to take. It also gave me exact products I could buy for the problem I had looked up. It makes my process SO MUCH easier when I need to find non-pharmaceutical solutions to my needs.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
