Bai Xian Pi

A long-standing staple skin herb in Chinese medicine, but human trial evidence is insufficient and liver injury has been reported.

Home

What is Bai Xian Pi? Bai Xian Pi is the dried root bark of Dictamnus dasycarpus, a plant in the rue family. In Traditional Chinese Medicine it is used for hot, red, itchy or weeping skin. It is a single herb, and it is almost always one part of a larger formula. Practitioners also use it for hives, damp skin sores and some joint aches with heat. It is boiled into a decoction (a water extract made by simmering). It is also used in washes and soaks for the skin. There are no good human trials of this herb on its own. Long use in a tradition is not proof that a herb works. Reports also link Bai Xian Pi to liver injury, so real care is needed. How does Bai Xian Pi work? The honest answer is that no one has shown how it works in people. What we have is laboratory and animal work. In test tubes and in animals, extracts appear to calm some immune and allergy signals. Some lab work also shows they slow the growth of certain fungi and bacteria. Whether any of that happens on human skin is not established. One lab finding matters for safety. Dictamnine, a main compound, appears to damage DNA in test tubes when UV light is present. Treat that as a warning flag, not a proven risk in people. What forms does Bai Xian Pi come in? Bai Xian Pi is sold as dried, sliced root bark for simmering into a decoction. Granules and powder extracts are also common. It is used on the skin too. Herbalists add it to washes, soaks and creams for itching. Tinctures (alcohol extracts) exist but are less common. It is rarely taken alone. It usually sits inside a multi-herb formula chosen for you. Quality and labelling vary a lot between suppliers. Where it comes from: Bai Xian Pi appears in Chinese medical writing from roughly the 5th century CE. Classical authors placed it with herbs said to clear heat and dry damp. It has been a staple skin herb in Chinese medicine ever since. It shows up in many classical formulas for rashes, sores and itching.

Helps

What does Bai Xian Pi help with? Eczema (atopic dermatitis): This is the main traditional use. Chinese medicine uses it in formulas for hot, red, itchy skin. We found no controlled human trial of Bai Xian Pi alone for eczema. Lab studies show some anti-allergy activity in cells. That is not the same as help for your skin. Hives (urticaria): Traditional use is long-standing, both taken by mouth and used as a wash. The evidence in people is not there. Hives that come with swollen lips, tongue or throat are an emergency. Do not treat those at home. General itchy skin: Classical texts rank it highly for itching from damp heat patterns. Modern trial support is absent. Itching has many causes, including liver and kidney problems. Lasting itch needs a proper check first. Fungal skin infections: In laboratory tests, extracts slow the growth of some fungi. Whether that works on human skin has not been shown. Proven antifungal creams are cheap and well tested. They are the sensible first choice. Weeping or oozing skin sores: Traditional use is as a wash or soak for damp, oozing rashes. No human trials support this use. Spreading redness, heat, pus or fever can mean infection. That needs medical care, not a herb wash. Joint pain with heat and swelling: Some classical formulas include it for hot, swollen joints. This use is less common than the skin uses. We found no human trial testing it for joint pain. The evidence here is traditional only.

Where it comes from:

Bai Xian Pi appears in Chinese medical writing from roughly the 5th century CE. Classical authors placed it with herbs said to clear heat and dry damp.

It has been a staple skin herb in Chinese medicine ever since. It shows up in many classical formulas for rashes, sores and itching.

If this is an emergency

Some reactions to Bai Xian Pi need urgent care. Call your local emergency number or go to an emergency department straight away — do not wait to see whether it passes.

Who should avoid Bai Xian Pi?

  • Do not use if you have any liver disease, hepatitis (swelling of the liver) or a past liver injury from a drug or herb
  • Do not use if you are pregnant or trying to become pregnant
  • Do not use if you are breastfeeding
  • Do not use if you take medicines that can harm the liver, such as methotrexate, isoniazid or regular paracetamol (acetaminophen)
  • Do not use if you have had an organ transplant or take anti-rejection drugs
  • Do not use if you drink alcohol heavily or daily
  • Do not use if you are giving it to a child, unless a doctor who knows this herb agrees
  • Do not use if you are allergic to rue family plants, such as rue or gas plant
  • Do not use if you have long-term loose stools and a weak, cold digestion, which classical texts treat as a reason to avoid it
  • Do not use if you cannot have liver blood tests checked when a doctor asks for them

When should you see a doctor?

Stop the herb and see a doctor the same day if your skin or the whites of your eyes turn yellow. Do the same for dark urine, pale stools, pain under your right ribs, feeling sick, or unusual tiredness. These can be signs of liver injury.

Get urgent care for swelling of the lips, tongue or throat, trouble breathing, or a rash with fever and blistering.

See your doctor if a rash spreads, weeps or smells bad, or if itching lasts more than two weeks. Tell any doctor you see that you took this herb.

Evidence grades and traditional-use labels are explained in our methodology. Nothing here is medical advice.

This app is so cool! I've always wanted there to be a hub for all NATURAL remedies and preventative options but l've always had to do loads of digging to find the info I wanted for different natural products. HealisticMD has all the info I need in one place. It gave me various natural supplemental options for a condition I looked up, including what each remedy does and how much is usually recommended to take. It also gave me exact products I could buy for the problem I had looked up. It makes my process SO MUCH easier when I need to find non-pharmaceutical solutions to my needs.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
