What is Xiao Luo Wan? Xiao Luo Wan means "Reduce Scrofula Pill". It is a classical Chinese herbal formula. It is sold as one finished product. Tradition uses it for soft or firm lumps in the neck. These include swollen lymph nodes and goitre (an enlarged thyroid gland). It is also used for other neck lumps. The word "formula" can mislead here. You take it as one drug: a single pill or granule. Three herbs are blended into it. The three are Xuan Shen (Scrophularia root), calcined Mu Li (oyster shell) and Zhe Bei Mu (Fritillaria bulb). Now the evidence. This is a true staple of Chinese medicine, with nearly 300 years of recorded use. But long use is not proof that it works. We can find no good human trials of this formula. So its evidence grade is insufficient. A lump in the neck always needs a diagnosis first. Neck lumps can come from infection, thyroid disease, tuberculosis or cancer. Some of these need urgent care. How does Xiao Luo Wan work? Nobody has shown how it works in people. The formula has not been properly tested in people. Chinese theory explains neck lumps as "phlegm" that thickens with heat. Xuan Shen is said to clear heat and soften hardness. Mu Li is said to soften lumps, and Bei Mu to transform phlegm. That is a traditional model. It is not a description of measured biology. Lab work gives some loose clues. Scrophularia root holds iridoid glycosides (plant compounds). These damp down inflammation (swelling) in cell and animal studies. Fritillaria holds alkaloids (plant chemicals) studied for cough and airway effects. Calcined oyster shell is mostly calcium carbonate. That is the same compound used in antacids (heartburn medicines). None of this has been tied to shrinking a lump in a person. Treat any mechanism you read as a hypothesis (an untested idea). What forms does Xiao Luo Wan come in? You will most often find it as: Small honey or water pills, the classical form. Concentrated granules or powder to mix into hot water. Capsules or tablets from modern makers. A raw decoction (a herbal tea). A practitioner weighs the three herbs and you simmer them. Buy only from suppliers who test each batch. They should check for heavy metals and for correct plant identity. Fritillaria bulbs have been mixed up with toxic look-alike bulbs. This happens in poorly controlled supply chains. Where it comes from: The formula comes from Yi Xue Xin Wu ("Medical Revelations"). Cheng Guo-Peng wrote it in 1732, during the Qing dynasty. It was aimed at scrofula, the old name for neck glands swollen by tuberculosis. It has stayed in steady use in China ever since. It is used across East Asian Chinese medicine too. It also appears in modern Chinese formula textbooks. Today its use is wider. It now covers thyroid nodules (lumps in the thyroid gland) and other neck lumps.

Helps

What does Xiao Luo Wan help with? Swollen or lumpy lymph nodes in the neck: This is the original target of the formula and its strongest traditional claim. There is no published human trial we can identify that tested it against placebo or against usual care. Swollen neck glands need a cause found first, because infection, tuberculosis and lymphoma all present this way. Goitre and thyroid nodules: This is the most common modern use, and it is where the formula is most often studied in Chinese-language reports. Those reports are mostly small, uncontrolled, and not verifiable to a standard we can cite. Thyroid nodules need ultrasound and blood tests, and some need a biopsy; herbs do not replace that. Firm lumps under the skin (traditional "phlegm nodules"): Traditional texts extend the formula to lipomas and similar soft-tissue lumps. This use rests on the traditional model of phlegm and heat, not on trial data. Any new or growing lump should be looked at by a doctor. Scrofula (tuberculous neck glands): The historical use, and the source of the formula's name. Tuberculosis today is treated with a specific combination of antibiotics taken for months. Using herbs instead of that treatment is dangerous and can let the infection spread.

You will most often find it as:

Small honey or water pills, the classical form.

Where it comes from:

The formula comes from Yi Xue Xin Wu ("Medical Revelations").

Who should avoid Xiao Luo Wan?

  • Do not use if you have a lump in your neck that a doctor has not yet examined and diagnosed.
  • Do not use if you are pregnant or trying to become pregnant.
  • Do not use if you are breastfeeding.
  • Do not use if you are allergic to shellfish, because the formula contains oyster shell.
  • Do not use if you are taking any aconite-containing Chinese formula, such as Fu Zi, Chuan Wu or Cao Wu — this pairing is never acceptable.
  • Do not use if you are being treated for tuberculosis or suspected tuberculosis in place of your prescribed antibiotics.
  • Do not use if you have hypercalcaemia (too much calcium in the blood) or kidney stones made of calcium.
  • Do not use if you take digoxin or another heart-rhythm medicine, unless your heart doctor has approved it.
  • Do not use if you have ongoing diarrhoea or a weak digestion that is worsened by cold, bitter herbs.
  • Do not use if you are buying an untested product with no heavy-metal screening, because oyster shell can carry lead or cadmium.
  • Do not use if you are giving it to a child, unless a qualified practitioner is supervising.

When should you see a doctor?

See a doctor before you start, so the lump gets a proper diagnosis with an examination, an ultrasound and blood tests.

Get urgent medical help if a neck lump is growing quickly, feels hard and fixed in place, or comes with night sweats, unexplained weight loss, fever or a hoarse voice.

Stop the formula and get seen if you develop a rash, swelling of the face or lips, trouble breathing, a slow or irregular heartbeat, dizziness, or diarrhoea that does not settle within a few days.

If your neck lump has not changed after six weeks, stop and go back to your doctor rather than continuing on your own.

Evidence grades and traditional-use labels are explained in our methodology. Nothing here is medical advice.

This app is so cool! I've always wanted there to be a hub for all NATURAL remedies and preventative options but l've always had to do loads of digging to find the info I wanted for different natural products. HealisticMD has all the info I need in one place. It gave me various natural supplemental options for a condition I looked up, including what each remedy does and how much is usually recommended to take. It also gave me exact products I could buy for the problem I had looked up. It makes my process SO MUCH easier when I need to find non-pharmaceutical solutions to my needs.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
