Zhi Zi Chi Tang

A long-standing staple of Chinese medicine (T2), but we found no human trials, so the evidence grade is insufficient.

What is Zhi Zi Chi Tang? Zhi Zi Chi Tang is a two-herb Chinese formula. In English it is called Gardenia and Prepared Soybean Decoction. It is made from dried gardenia fruit (Zhi Zi) and fermented black soybean (Dan Dou Chi). Chinese medicine uses it for what it calls leftover heat in the chest. In plain words: feeling hot, edgy and unable to sleep after a fever has passed. It is also taken for a tight, uneasy feeling in the chest. This library treats the whole formula as a single drug entry. The evidence is thin: we could find no human trials of this formula. It has been used for about 1,800 years , but long use is not proof that a remedy works. How does Zhi Zi Chi Tang work? No one has shown how this formula works in people. What follows is the traditional idea, plus some lab findings. In Chinese medicine, gardenia fruit is seen as cold and bitter. It is used to clear heat that leaves a person restless. Fermented soybean is seen as light and mild, and is used to lift leftover heat out to the surface. In lab dishes and in animals, geniposide (the main gardenia compound) lowers markers of swelling. Some animal work also points to a calming effect. That work uses pure compounds, not the herb tea, so it does not show the same thing happens in you. What forms does Zhi Zi Chi Tang come in? The classic form is a decoction. You boil the two herbs in water and drink the warm liquid. You can also buy it as granules that melt in hot water, or as pills or capsules made from dried extract. These are easier to take but the strength varies from brand to brand. Some shops sell the two herbs loose so you can cook them at home. Quality varies, and gardenia fruit is sometimes mixed up with other fruits. Buy from a seller who tests what they sell. Where it comes from: The formula comes from the Shang Han Lun (Treatise on Cold Damage), a Chinese medical text from about 200 CE. Its author, Zhang Zhongjing, wrote it for restlessness and poor sleep that hung on after a fever. The same old text gives a warning. If the drink makes you vomit, stop taking it. It is a small, plain formula and has stayed in use ever since. Later doctors added herbs to it for related problems. It is still taught in Chinese medicine schools today.

Helps

What does Zhi Zi Chi Tang help with? Restlessness and irritability after a fever: This is the main traditional use, and the one the original text describes. Classical doctors gave it when a fever had broken but the person still felt hot, edgy and unable to settle. We found no human trial testing this use. Trouble sleeping that follows an illness: Tradition pairs the sleep problem with the restless, hot feeling above, not with long-term insomnia. There is animal work suggesting gardenia compounds have a calming effect. That is not the same as showing it helps you sleep. A tight, stuffy feeling in the chest with no pain: Old texts describe a soft, uneasy fullness under the breastbone that feels worse when lying down. Chest tightness has many causes, and some are serious. Get chest symptoms checked by a doctor before you treat them yourself. A low fever that lingers after the main illness has passed: Traditionally used for mild heat left over at the end of an illness, not for a high or new fever. No human trial supports this. A fever that lasts more than three days needs a doctor, not a herb. Feeling hot and flushed in the evening: Practitioners use it for a warm, agitated feeling that rises at night. This sits entirely on tradition. We found no trial evidence for it.

This library treats the whole formula as a single drug entry. The evidence is thin: we could find no human trials of this formula. It has been used for about 1,800 years, but long use is not proof that a remedy works.

Where it comes from:

The formula comes from the Shang Han Lun (Treatise on Cold Damage), a Chinese medical text from about 200 CE.

Who should avoid Zhi Zi Chi Tang?

  • Do not use if you have liver disease (a damaged or poorly working liver), hepatitis, or a blood test showing raised liver enzymes
  • Do not use if you take warfarin, apixaban, clopidogrel or any other blood thinner, unless your doctor agrees and checks you
  • Do not use if you take an MAO inhibitor such as phenelzine or tranylcypromine (older antidepressants), because fermented soy can contain tyramine and push your blood pressure up sharply
  • Do not use if you are pregnant or trying to get pregnant: safety has not been shown and gardenia is a strong, cold, bitter herb
  • Do not use if you are breastfeeding
  • Do not use if you are allergic to soy
  • Do not use if you have long-term diarrhoea, loose stools, or a weak digestion that gets worse after cold food or drink
  • Do not use if you are taking another gardenia-based formula at the same time
  • Do not use if you are giving it to a child, unless both a qualified Chinese medicine practitioner and your doctor agree
  • Do not use if you drink heavily or already take a medicine that is hard on the liver, such as paracetamol (acetaminophen) or methotrexate

When should you see a doctor?

Stop taking it and see a doctor the same day if your skin or the whites of your eyes turn yellow, your urine goes dark, or you get pain under your right ribs. These can be signs of liver trouble.

See a doctor promptly if a fever lasts more than three days, if you keep vomiting, or if diarrhoea goes on for more than two days.

Get urgent help if you have chest pain or pressure, trouble breathing, black or bloody stools, or a sudden pounding headache. Chest tightness and a bad headache can both come from causes that need fast treatment.

This app is so cool! I've always wanted there to be a hub for all NATURAL remedies and preventative options but l've always had to do loads of digging to find the info I wanted for different natural products. HealisticMD has all the info I need in one place. It gave me various natural supplemental options for a condition I looked up, including what each remedy does and how much is usually recommended to take. It also gave me exact products I could buy for the problem I had looked up. It makes my process SO MUCH easier when I need to find non-pharmaceutical solutions to my needs.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
