What is Shakuyakukanzoto? Shakuyakukanzoto is a traditional two-ingredient herbal formula consisting of white peony root (Shakuyaku) and licorice root (Kanzo). It originates from the classical Chinese medical text Shang Han Lun (circa 200 CE) and was later adopted as a cornerstone of Japanese Kampo medicine. Today it is one of the most widely prescribed Kampo formulas in Japan, available as a standardized extract through major manufacturers like Tsumura. How does Shakuyakukanzoto work? The formula works through complementary pathways: peony root's main compound paeoniflorin relaxes smooth and skeletal muscle by modulating calcium signaling and reducing excitatory nerve activity. Licorice root's glycyrrhizin compounds have anti-inflammatory and antispasmodic effects and also enhance the absorption and bioavailability of paeoniflorin. Together, they calm overactive muscle contraction more effectively than either herb alone. What forms does Shakuyakukanzoto come in? In Japan, standardized granule extract sachets (TJ-68 by Tsumura) are the most clinically studied and commonly prescribed form, typically dosed as 2.5 g granules three times daily. Traditional decoctions and capsules are also available outside Japan. When choosing a product, look for standardized extracts that specify paeoniflorin content. Where it comes from: The formula appears in Zhang Zhongjing's Shang Han Lun, one of the foundational texts of Chinese medicine, written around 200 CE. It was introduced to Japan and refined within the Kampo system over centuries. In modern Japan, Kampo formulas including Shakuyakukanzoto are covered by national health insurance and routinely prescribed by physicians, making Japan unique globally in integrating classical herbal formulas into mainstream medical practice.

Helps

What does Shakuyakukanzoto help with? Muscle cramps and spasms: This is what Shakuyakukanzoto is most famous for. It relaxes overactive muscles quickly by calming the nerve signals and calcium activity that trigger a cramp. Many people feel relief within 20–30 minutes of taking a dose, and it works for both skeletal muscles (like calves) and smooth muscles (like the gut or uterus). Nighttime leg cramps: Nocturnal leg cramps are a common and disruptive problem, especially in older adults and people with liver disease. Shakuyakukanzoto has been studied specifically for this use and has shown meaningful reductions in how often cramps occur and how severe they are. Menstrual cramps and period pain: By relaxing the smooth muscle of the uterus and reducing inflammation, this formula can ease the painful contractions behind period cramps. It is used in Japanese gynecology as a gentler alternative or complement to conventional pain relief. Irritable bowel syndrome (IBS) and gut spasms: The antispasmodic action of the formula also extends to the digestive tract, helping to ease cramping abdominal pain and the uncomfortable spasms that often accompany IBS. It calms the gut without significantly slowing digestion overall. Polycystic ovary syndrome (PCOS): Research — particularly from Japan — suggests Shakuyakukanzoto may help women with PCOS by reducing testosterone levels and supporting more regular ovulation. Paeoniflorin appears to interfere with enzymes involved in androgen production in the ovaries. Chemotherapy-induced muscle cramps: Cancer patients receiving certain chemotherapy drugs (especially oxaliplatin) frequently experience severe muscle cramps. Clinical studies in Japan have shown this formula can significantly reduce both the frequency and intensity of these cramps, improving quality of life during treatment. Cirrhosis-related muscle cramps: People with liver cirrhosis frequently suffer from painful leg cramps. Multiple Japanese clinical trials have specifically evaluated Shakuyakukanzoto in this population, finding it reduces cramp frequency and severity with a good safety profile even in those with compromised liver function. Low back pain with muscle tension: When back pain involves tight, spasming muscles rather than structural injury, this formula's muscle-relaxing properties can provide meaningful relief. It is sometimes used in Kampo practice alongside other formulas for this purpose.

This app is so cool! I've always wanted there to be a hub for all NATURAL remedies and preventative options but l've always had to do loads of digging to find the info I wanted for different natural products. HealisticMD has all the info I need in one place. It gave me various natural supplemental options for a condition I looked up, including what each remedy does and how much is usually recommended to take. It also gave me exact products I could buy for the problem I had looked up. It makes my process SO MUCH easier when I need to find non-pharmaceutical solutions to my needs.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
