Orento

A long-standing staple of Kampo with centuries of recorded use, but human trial evidence is insufficient and any benefit remains unproven.

What is Orento? Orento is a traditional Japanese Kampo herbal formula taken for an upset stomach — burning or hot discomfort high in the belly, along with nausea and vomiting. It is given as a single Kampo formula, meaning one prescription used on its own rather than mixed with other Kampo formulas. It is also known as Coptis decoction, after its lead herb. Kampo practitioners reach for it in gastritis (irritation of the stomach lining), indigestion, and belly upset that comes with a bad taste, mouth soreness or loose stools. Here is the honest verdict up front: Orento has centuries of recorded use, but good human trials testing it are essentially missing. Traditional standing tells you people have relied on something for a long time; it is not proof that it works. How does Orento work? No one has shown exactly how Orento works in the body. In Kampo thinking, it is described as clearing "heat" in the stomach while warming the middle of the body and settling the gut. That is a traditional model of the body, not a tested mechanism. In laboratory work, berberine from Coptis calms inflammation and slows the growth of some gut bacteria. Ginger and pinellia have shown anti-nausea (anti-vomiting) effects in animals. Whether any of this happens at the small amounts found in a daily dose is unknown. What forms does Orento come in? Most people in Japan take Orento as a dried extract granule, sold in single-dose sachets under names such as TJ-120 or Kracie 120. Some brands press the same extract into tablets or capsules. The older form is a decoction: the raw sliced herbs simmered in water and strained, then drunk warm. This is prepared by a practitioner or pharmacy rather than bought off a shelf. Products vary in strength between makers, so follow the label on the box you actually have. Where it comes from: Orento comes from the Shang Han Lun (Treatise on Cold Damage), a Chinese medical text compiled around 200 CE, where it is called Huang Lian Tang. Japanese doctors adopted it into Kampo, the Japanese school of herbal medicine, and it has stayed in continuous use there for centuries. Today it is an approved prescription extract in Japan and is covered by national health insurance.

Helps

What does Orento help with? Gastritis (irritation of the stomach lining): This is the classic Kampo reason for prescribing Orento, and the use named on Japanese product labels. The support is traditional and clinical habit, not trial data. We could not find a controlled human trial of Orento for gastritis. Nausea and vomiting with upper-belly burning: Kampo texts describe Orento for a hot, burning feeling high in the belly that comes with nausea and throwing up. Ginger and pinellia in the mix are the traditional anti-nausea (anti-vomiting) herbs. Their effect has been seen mainly in animal and laboratory studies, not in trials of this formula. Indigestion (dyspepsia): Practitioners use it for slow, uncomfortable digestion with belching and a bad taste in the mouth. Evidence is traditional. If your indigestion is new, severe, or comes with weight loss, see a doctor rather than self-treating. Mouth ulcers and a sore, bitter-tasting mouth: Some Japanese practitioners use Orento for mouth soreness, and a related formula has been studied more for this. Studies of that related formula do not transfer to Orento. For Orento itself, this use rests on tradition. Loose stools that come with stomach upset: Coptis has a long record of use for loose stools across East Asian traditions. There is laboratory work on berberine and gut bacteria, but no human trial of Orento for this. Diarrhoea lasting more than a few days needs medical assessment.

Where it comes from:

Orento comes from the Shang Han Lun (Treatise on Cold Damage), a Chinese medical text compiled around 200 CE, where it is called Huang Lian Tang. Japanese doctors adopted it into Kampo, the Japanese school of herbal medicine, and it has stayed in continuous use there for centuries. Today it is an approved prescription extract in Japan and is covered by national health insurance.

Who should avoid Orento?

  • Do not use if you have aldosteronism (a hormone condition that raises blood pressure and lowers potassium).
  • Do not use if you have low potassium in your blood, or have been told your potassium runs low.
  • Do not use if you have myopathy (a muscle disease causing weakness), including the kind caused by licorice.
  • Do not use if you are taking another licorice-containing Kampo formula or a glycyrrhizin medicine, unless a doctor has added up the total for you.
  • Do not use if you take a diuretic (water pill) or digoxin (a heart drug) without your doctor's agreement.
  • Do not use if you have heart failure, poorly controlled high blood pressure, or serious kidney or liver disease, unless a doctor supervises it.
  • Do not use if you are pregnant or breastfeeding, unless a doctor who knows Kampo advises it.
  • Do not give it to a child except on the advice of a doctor.
  • Do not keep taking it if you develop a rash, swelling, or muscle weakness — stop and get checked.

When should you see a doctor?

Stop taking Orento and see a doctor the same day if you notice yellow skin or eyes, dark urine, or deep unexplained tiredness — these can be signs of liver injury. Do the same for a new dry cough with fever and breathlessness, which can be a sign of lung inflammation.

Get urgent help for muscle weakness, bad cramps, a fluttering or irregular heartbeat, or sudden swelling in the face, ankles or hands. These can mean your potassium has dropped too low.

See a doctor before starting if your stomach pain is severe, if you are vomiting blood or passing black tarry stools, or if you have lost weight without trying. Also book a check if you take Orento for more than about a month, so your blood pressure and potassium can be tested.

This app is so cool! I've always wanted there to be a hub for all NATURAL remedies and preventative options but l've always had to do loads of digging to find the info I wanted for different natural products. HealisticMD has all the info I need in one place. It gave me various natural supplemental options for a condition I looked up, including what each remedy does and how much is usually recommended to take. It also gave me exact products I could buy for the problem I had looked up. It makes my process SO MUCH easier when I need to find non-pharmaceutical solutions to my needs.

There is a wealth of information on the app! I recently had bloodwork done and found I was deficient in a handful of vitamins and minerals (Vitamin B12, Vitamin D, Iron, etc.) HealisticMD made it simple for me to find out what foods to eat/ supplements I needed to improve these deficiencies. It gives you so many ideas on prevention, natural health tips, where to buy the vitamins, when to take them, why, etc. I love how easy the app is to use too. I am currently working on getting pregnant. I was pleasantly surprised to find compelling data along with supplement suggestions to aid in the process, some of which my fertility doctor wasn’t even aware of. Highly recommend downloading, after all health is one our greatest gifts!
